Overview

Urgent, Inc.'s Youth Empowerment After School ProgramUrgent, Inc.'s Youth Empowerment After School Program provides a low cost, high quality, fun, safe and enriching after school program for boys and girls in grades 2nd-5th utilizing best practice strategies which support national out-of-school time program standards during the school year.

When: School year 2009-2010

Where: Goulds Elementary School 23555 SW 112th Ave. Miami, FL 33170, rooms 315-316

Who: Boys and Girls in grades 2nd-5th

Time: Mon-Fri after school until 6:00pm

Registration: $30.00/per Child or $40.00 per Family

All registration are Non-Refundable

Fees: Only $4/child per day or Discount $2.00/child per day rate for Parents who attend monthly Parent meetings and Volunteer a minimum of 1 hour per month. Money Orders ONLY or online at www.urgentinc.org

Family Involvement: Monthly Parent Skills Training and Monthly Volunteer opportunities including Parents and Children Reading Together Time, Family Arts & Crafts and Family Fitness Time

Children's TrustThe Youth Empowerment Program is funded by The Children’s Trust. The Children’s Trust is a dedicated source of revenue established by voter referendum to improve the lives of children and families in Miami-Dade County by making strategic investments in their future.

There are several steps to officially enroll your child in the Youth Empowerment After School Program.

1) Complete Enrollment Application for 2009-2010 school year. The forms can be returned directly to: Emily Gunter, Program Site Manager @ Goulds Elementary. If Emily is not on campus you can drop it off in the office. Make sure they put your application in the Urgent, Inc. After School Program mailbox. Staff will be at Goulds Elem. Beginning on August 17th -21st from 1:00 PM-4:00 PM. Download Forms Here

2) Pay Registration Fee and 1st two service period fees prior to August 24th to ensure you child can begin on the first day of school. Registrations are taken on a first come first serve and are non-refundable. Before you pay, please call to verify we are still accepting applications for your child’s age group.

3) Attend one Mandatory Parent Orientation. Parent orientations will be held daily during the week of August 17-24, 2009 from 2:00-4:00 PM.

Your child will not be able to begin the program until all enrollment activities have been completed successfully.

REGISTRATION & WEEKLY FEES

Girl

After School Fees:
Fees must be paid in advance on a monthly basis. Below is a fee payment schedule for the After-School Program. Please note the due dates, services dates and amount due. There will be a charge of $10.00 if fees are not made by the first day of the new service period.

No child will be allowed to begin a service period for which payment has not made prior to the first day of the next service period.

Also, consistent late payments will result in the removal of your child from the program. Fees may be paid by money order or online at www.urgentinc.org. There is no partial payment for partial use of services.

Fees:

  • Registration: $30/per child or $40/per family
  • Daily Fee $4/per child
  • Discounted Daily Fee $2/per child- based on The Children’s Trust funded slot and monthly participation in Parent Trainings and Volunteering a minimum of 1 hour per month
  • After 6:00pm pick-up $5 for every 15 minutes

Discount Rate Eligibility: Parent Training and Volunteer Commitment

Parents must attend 1 monthly training and volunteer a minimum of 1 hour a month to become eligible to receive the reduced monthly rate of $2 day per child. In order to become eligible to volunteer with the program parents must complete the MDCPS volunteer registration form and submit with your child’s enrollment package. Parents must sign in at each parent meeting and volunteer time to verify eligibility for the reduced rate the following service month.

Please note: the first 39 slots of this program are subsidized by The Children’s Trust. Children enrolled after the first 39 Children’s Trust Slots are not eligible to receive the discounted rate. The Registration fee will remain the same.
