When: School year 2008-2009 Where: Goulds Elementary School 23555 SW 112th Ave. Miami, FL 33170 Who: Boys and Girls in grades 2nd-5th Time: Mon-Fri after school until 6:00pm Registration: $30.00/per child; 9-Week Session Fee $20/per child Highlights: Daily Snack, Quarterly Field Trips, Music, Arts and Crafts, Story Time, Homework Help, Social Skills, Martial Arts, Math and Science Enrichment, Recreation, End of School Talent Exhibition Best Practices: SPARK- Sports Play and Recreation for Kids After School, Peace Education, Life Skills Training, After School KidzLit Family Involvement: Mandatory monthly parent skills training; Monthly Parent and Child Book Club (Optional) Program Contact: Emily Gunter, Site Coordinator 305-915-3195 or emily@urgentinc.org EnrollmentThere are several steps to officially enroll your child in the Youth Empowerment After School Program. 1) Complete Enrollment Application for 2008-2009 school year (Click here for forms). The forms can be returned via mail to: Urgent, Inc. 1600 NW 3rd Ave. Bldg Miami, FL 33136, Faxed to 305-576-2089 or can be dropped off to Goulds Elementary. Make sure they put your application in the Urgent, Inc. After School Program mailbox. 2) Pay Registration Fee. Registrations are taken first come first serve and are non-refundable. Before you pay, please call to verify we are still accepting applications. 3) Attend a Mandatory Parent Orientation. Parent orientations will be held in May 2008 and again starting July 2008. Call for dates, time and location. Your child will not be able to begin the program until all enrollment activities have been completed successfully. Registration (Non-Refundable) Must be paid prior to start of school year 2008-2009 Nine-Week After School Fee (Non-Refundable) Must be paid prior to start of 9-week session.
