Overview

Urgent, Inc.'s Youth Empowerment After School ProgramUrgent, Inc.'s Youth Empowerment After School Program provides a low cost, high quality, fun, safe and enriching after school program for boys and girls in grades 2nd-4th utilizing best practice strategies which support national out-of-school time program standards during the school year.

When: School year 2010-2011

Where: Goulds Elementary School 23555 SW 112th Ave. Miami, FL 33170, rooms 315-316

Who: Boys and Girls in grades 2nd-4th

Time: Mon-Fri after school until 6:00pm

Supply Contribution: $30.00/per Child or $40.00 per Family - Due with application

All registrations are Non-Refundable

Snack Processing & Consumable Supplies Contribution: Only $4/child per day.

Children's TrustThe Youth Empowerment Program is funded by The Children’s Trust. The Children’s Trust is a dedicated source of revenue established by voter referendum to improve the lives of children and families in Miami-Dade County by making strategic investments in their future.

Enrollment Process

There are several steps to officially enroll your child in the Youth Empowerment After School Program.

1) Complete Enrollment Application for 2010 - 2011 school year. The forms can be returned directly to: Emily Gunter, Program Site Manager @ Goulds Elementary. If Emily is not on campus you can drop it off in the office. Make sure they put your application in the Urgent, Inc. After School Program mailbox. Staff will be at Goulds Elem. Beginning on August 23rd from 1:00 PM. Download Forms Here

2) Pay Contribution and 1st service period prior to August 23rd to ensure you child can begin on the first day of school. Enrollments are taken on a first come first serve and contributions are non-refundable. Before you pay, please call to verify we are still accepting applications for your child’s age group.

3) Attend one Mandatory Parent Orientation. Please contact Site Manager Emily Gunter to confirm the time and location at 305-915-3195.

Your child will not be able to begin the program until all enrollment activities have been completed successfully.

REGISTRATION & FINANCIAL CONTRIBTION

After School Fees:
Fees must be paid in advance on a monthly basis and are due no later than the 1st of each month.

Consistent late payments will result in the removal of your child from the program. Fees may be paid by money order or online at www.urgentinc.org. There is no partial payment for partial use of services.

Late Pick-Up:
Late pick-up is considered anytime after 6:10pm. The cost of late pick-up is $10. This must be paid at the time of pick-up.

MANDATORY PARENT INVOLVEMENT: Parent Training & Education with MDCPS Parent Success Academy

Parents must attend one (1) monthly training on the 3rd Tuesday of each month from 5:00-6:30pm to maintain enrollment in the program.

Registration (Non-Refundable) and fess for the first service period must be paid prior to August 23, 2010
